Details Home health setting serving adult patients across Racine and Kenosha Counties in WI Day shift schedule, Monday–Friday, approximately 7:45 AM–4:00 PM Caseload of 6 patients/points per day — evaluation visits count as 1.5 points, standard visits count as 1 point Patient population is strictly adult home health, including geriatric, post-stroke, post-joint replacement, weakness/deconditioning, COPD, and CHF cases Most patient visits are within 20–30 minutes of each other; occasional visits may be up to approximately one hour away EMR: Epic (experience preferred) No weekend rotation, no on-call, and no holiday coverage required Requirements Active WI OT license required if applicable Current BLS certification required Minimum 3 years of OT experience in SNF, home health, or acute care; home health experience preferred Bachelor's degree in Occupational Therapy accepted Valid driver's license, personal vehicle, and active auto insurance required General OT treatment and evaluation skills with a focus on ADLs and adaptive equipment Epic EMR experience preferred Minimum 1 professional reference from a Supervisor, Manager, Director, or Charge RN dated within the last year Candidate DOB and last 4 digits of SSN required for consideration Additional Information First-time travelers are welcome to apply Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months to be eligible Local candidates are accepted at the same rate — no radius restrictions apply Orientation begins at the National Avenue office in Milwaukee, includes an Epic training class, and is followed by a full week with a preceptor Daily morning team huddles conducted via phone with the interdisciplinary team Works alongside an interdisciplinary team including RNs and PTs, supported by an RN supervisor, PT lead, PT manager, and other OTs Dress code is business casual or any color scrubs Manager interview required prior to any offer being extended RTO requests must be approved by the manager American Traveler Job ID P-702550.
