Day Shift - 7.5 Hours (United States of America)

The HIV Patient Navigator provides patient-centered education, advocacy, and care coordination services to individuals living with HIV. This role supports patients in navigating the healthcare system, addressing barriers to care plan adherence, and facilitating access to clinical and community-based services. Working collaboratively with the healthcare team under the leadership of a Nurse Practitioner, the HIV Patient Navigator plays a key role in improving health outcomes and supporting continuity of care.

Essential Duties And Responsibilities
  • Provide education, guidance, and emotional support to patients who are HIV positive.
  • Identify and assist in addressing barriers to care plan adherence, including but not limited to access to services, appointment follow-up, and psychosocial needs.
  • Coordinate referrals to internal departments and external community resources as appropriate.
  • Support transitions of care to ensure continuity between inpatient, outpatient, and community settings.
  • Serve as a liaison between patients and the clinical care team to promote patient engagement and retention in care.
  • Assist with documentation and tracking of patient interactions in electronic systems.
  • Participate in team meetings and contribute to care coordination initiatives.
  • Initiate and support projects related to patient navigation and quality improvement.
  • Maintain patient confidentiality and comply with all regulatory and organizational policies.
Education
  • High School Diploma or equivalent required.
Experience
  • Prior experience working with patients.
  • Experience in a hospital or community health setting preferred.
  • Experience working with the HIV-positive population or similar patient populations preferred.
Skills & Abilities
  • Computer literacy required, including use of electronic health records.
  • Ability to work collaboratively within a multidisciplinary healthcare team.
  • Strong interpersonal, communication, and organizational skills.
  • Ability to take initiative, start projects, and work effectively under the leadership of a Nurse Practitioner.

Salary Range: $27.00/hr - $35.00/hr

Wednesday, Thursday & Friday 8:00 AM – 4:00 PM
