Description

The Patient Advocate serves as a bridge between the patient and their healthcare provider, providing high-quality patient-centered care. As a Patient Advocate your primary role is to support patients and help them overcome barriers to receiving the healthcare services and medication they need. The primary responsibilities include medication assistance, benefit investigation, billing, medical record inquiries, and prior authorizations. The patient advocate will also assist the clinical pharmacist with patient outreach and to coordinating timely deliveries, patient consultations, among other duties as required. Preferred candidates will have at least two-years of pharmacy technician experience with a demonstrated commitment to providing high-quality patient care. Responsibilities
  • Benefit Investigation: Using the electronic medical record and other platforms to assist providers with the completion of prior authorizations for specialty medications, as well as following up with insurance companies for approval status.
  • Specialty Medication Refills: Calling patients to assess adherence, adverse reactions, verifying patient demographics, processing refills, and scheduling future outreach.
  • Financial investigation: Verifying copays and finding financial resources to make specialty medication more affordable. Guiding patients and offices through copay assistance and Patient Assistance Programs as applicable.
  • Documentation: Thoroughly document all communications and updates pertinent to each step of the program within the specialty pharmacy management software.
  • Collaborate with providers: working with provider practices to keep refills active, prior authorizations up to date, and patient electronic medical records current.
  • On Site Work: creating welcome packets and mailing out to patients, as well as other documents as needed and inventory control of these supplies. Retrieving Specialty mail and sending to required departments.
  • Triaging Patients: assessing patients' needs and triaging to Pharmacist as needed.
  • Maintain compliance with NLH Remote Worker Policy: Remote Work (17-104).
  • Other Duties as needed.
