Job Description
Specific responsibilities include being the go-to resource for the person served and their family, assuring that effective safety net resources are in place, clearly communicating expectations, assuring periodic assessment and cross system planning, preparing for, convening and facilitating service planning meetings, assuring cross system coordination of services, developing enduring relationships through persistent outreach, assisting the person served in developing and using natural supports, being a persistent advocate for those served, providing a consistent positive outlook which encourages recovery and full inclusion in the community, attending agency, county and state training as appropriate and/or required, providing comprehensive assessment of consumer strengths and needs in all life domains, completing and implementing strengthsābased service plans, actively participating as part of the treatment team, providing crisis support, advocating for consumer and/or family choice of services, utilizing recovery principles in efforts to promote consumer self-management of illness, providing advocacy for consumers involved in 302 and other involuntary proceedings, maintaining minimum required face-to-face contacts with consumers, completing and documenting assessments, service plans, progress notes and other required documentation in a timely manner, participating in 24 hour on-call rotation, providing phone and face-to-face contacts after business hours, being on site for all in-patient MH commitments, ensuring that all alternatives to hospitalization have been considered, coordinating intervention when hospitalization is required.
Accountabilities include completing the responsibilities of the position accurately and efficiently, ability to describe program and communicate purpose of services to consumer/family in clear manner and clarifies case manager and consumer expectation of services, conviction about the capacity of people to grow and change, establishing a respectful relationship with persons served to help them gain skills and confidence, effective use of assessment skill to determine client needs and appropriate interventions, ability to articulate clear measurable service plan goals that are identified by consumer/family and are reviewed and revised as consumer circumstances dictate, accurately complete all required documentation for service coordination services within the designated time frames, knowledge of all program policies/procedures, meeting productivity standards, maintaining professional boundaries and ethics, maintaining appropriate dress and grooming for the position as determined by the supervisor, interacting effectively with clients, families, clients' treatment team, other staff and external contacts, analyzing complicated issues and solving problems while involving supervisor appropriately, navigating, negotiating and advocating within multiple service systems on behalf of consumers while maintaining professionalism and amicable professional relationships, knowledge of crisis prevention and management strategies, comprehensive knowledge of mental illness and addictive disorder and best practice treatment modalities including psychopharmacology, ability to coordinate and communicate with client's treatment team to effectively orchestrate continuity of care, ability to integrate CASSP, CSP, and Recovery Principles into all aspects of service delivery, knowledge of theory based clinical skills that are effectively applied to supporting consumers' recovery process.
Required minimum qualifications include a Bachelors' Degree in social work, psychology, criminal justice, or education for Child/Adolescent Service Coordination, three years of Service Coordination/Case-management related experience or five years mental health related experience, complete the CANS/ANSA Certification within 6 weeks of employment, valid Pennsylvania Driver's License, Act 33 and 34 Clearances, must have reliable vehicle to utilize for transportation when agency vehicles are unavailable, additionally, Child/Adolescent Service Coordination requires current FBI clearances (less than 1 year old).
