Position Summary
The Care Coordinator plays a critical role in ensuring clients receive consistent, high-quality care. This position is responsible for caregiver scheduling, client communication, and coordinating services between caregivers, clients, and management.
This role requires strong organization, communication, and problem-solving skills.
Key Responsibilities
Coordinate and manage caregiver schedules and client assignments.
Ensure all client shifts are covered and staffed appropriately
Communicate with caregivers regarding schedule updates, call-offs, and replacements
Serve as a point of contact for caregivers and clients
Monitor caregiver attendance and shift completion through scheduling systems
Assist with Intake of new clients and field visits according to schedule.
Maintain accurate documentation of client services in HHA
Respond to urgent staffing issues and find coverage when necessary
Work closely with management to ensure high-quality service delivery
Support general administrative tasks as needed
Qualifications
Experience in home care, medical scheduling, or care coordination preferred
Strong organizational and multitasking skills
Excellent communication and interpersonal skills
Ability to handle sensitive and confidential information
Experience using scheduling software or healthcare systems (experience with HHAeXchange is a plus)
Proficiency in Google Workspace and email communication.
Ability to remain calm and solutions-focused in a fast-paced environment.
