Position Overview

CMA Coordinator – Ongoing

Department: Case Management Reports to: Teams Supervisor FLSA Status: Non-Exempt Supervisor Responsibilities: None

The CMA Coordinator plays a critical role in helping individuals access and coordinate long-term care services in a complex and highly regulated environment.

In this role, you will build meaningful relationships with individuals, families, and community partners while ensuring services are effectively planned, coordinated, and monitored. Your work directly supports the health, safety, independence, and well-being of the people we serve.

Key Responsibilities

  • Service Coordination
    • Coordinate and develop person-centered service plans for individuals receiving long-term care services
    • Communicate regularly with individuals, families, providers, and community partners
    • Ensure services meet individual needs while maintaining compliance with regulations and policies
  • Monitoring and Advocacy
    • Monitor service delivery and utilization to ensure quality care
    • Advocate for individuals to promote autonomy, safety, and independence
    • Connect individuals and families to community resources
  • Compliance and Documentation
    • Maintain accurate records in state and internal systems
    • Stay current on Long-Term Care (LTC) regulations and requirements
    • Complete service coordination tasks using digital tools and databases
  • Community Engagement
    • Meet with individuals in homes, community locations, and facilities
    • Participate in departmental meetings, trainings, and collaborative initiatives
    • Travel locally to support the individuals we serve
  • Additional Responsibilities
    • Provide coverage for team members as needed to maintain continuity of services
    • Participate in professional development and goal setting
    • Adapt to evolving state and federal regulations affecting long-term care services
    • Support agency initiatives that promote the health and success of TRE and the people we serve

Experience with:

  • Microsoft 365 (Word, Excel, Outlook, Teams, SharePoint)
  • Databases and electronic records
  • Mobile devices and tablets
  • Virtual meeting platforms

Education & Experience

Minimum Requirements

  • Bachelor's degree in any field OR
  • 5 years of experience working with vulnerable populations, individuals with disabilities, or in a non-profit/human services setting OR
  • A combination of education and relevant experience working with vulnerable populations or individuals with disabilities

Preferred

  • Knowledge of Medicaid Waiver programs
  • Experience working with individuals with disabilities
  • Familiarity with community resources and service systems

Additional Requirements

  • Reliable transportation for community visits
  • Valid Colorado Driver's License (unless active military family)
  • Minimum auto insurance coverage of $100,000 / $300,000 / $100,000 liability
