Patient Care Coordinator – Internal & Family Medicine

Unity Health Care | Washington, DC Job Type: Full-Time | FLSA Status: Non-Exempt Reports To: Health Center Director

Position Summary

Unity Health Care is seeking a Patient Care Coordinator – Internal & Family Medicine to support vulnerable and medically complex patients through care coordination, outreach, and patient navigation. This role collaborates with providers, nurses, and social service staff to ensure coordinated, patient-centered care across clinics, hospitals, and community settings.

Key Responsibilities
  • Coordinate care and provide patient navigation for high-risk patients
  • Support development and follow-up of individualized care plans
  • Coordinate services with hospitals, providers, specialists, and community organizations
  • Track patients following hospital discharge or emergency visits and ensure follow-up care
  • Assist patients with insurance, transportation, translation, and other barriers to care
  • Schedule appointments and coordinate specialty referrals
  • Conduct outreach in clinics, hospitals, homes, and community settings
  • Identify gaps in preventive and chronic care services
  • Participate in care team meetings and case conferences
  • Ensure compliance with HIPAA and Unity Health Care policies
Minimum Qualifications
  • High School Diploma or GED required
  • College coursework in healthcare, business, or related field preferred
  • 2+ years of care coordination or patient navigation experience
  • Experience in hospital, outpatient, or community health settings preferred
  • Experience working on interdisciplinary healthcare teams
Required Skills
  • Knowledge of medical terminology and ICD-10
  • Familiarity with community health, discharge planning, and chronic disease management
  • Strong communication, organizational, and interpersonal skills
  • Proficiency with Microsoft Office and Electronic Medical Records (EMR)
  • Ability to work with diverse patient populations and travel between DC clinic locations
