Admissions & Census Coordinator

Are you a relationship-builder who thrives in a fast-paced healthcare environment? We are seeking a driven and personableAdmissions & Census Coordinator to help grow and support our community by guiding residents and families through the admissions process while building strong referral partnerships.

In this role, youll be at the center of our growthconnecting with hospitals, families, and community partners to ensure a smooth transition for new residents while helping us maintain a strong and stable census.

What Youll Do
Lead the admissions process from initial inquiry through move-in, ensuring a seamless and positive experienceManage census development goals, including admissions, discharges, and occupancy trackingVerify insurance/benefits and coordinate with internal teams for efficient placementBuild and maintain referral relationships with hospitals, physicians, and community partnersGenerate and track leads, analyze trends, and report on census activityRepresent the community at networking events, outreach efforts, and marketing initiativesCommunicate effectively with residents, families, and interdisciplinary team membersMaintain accurate documentation, reports, and compliance with all regulatory standardsSupport surveys, audits, and continuous quality improvement efforts

Education & Experience
Clinical background preferred: RN, LPN, or Social WorkAssociate or Bachelors degree preferredExperience in healthcare admissions, case management, or census development is a plus

Licensure
Active nursing license (preferred)
