Admissions & Census Coordinator

Are you a relationship-builder who thrives in a fast-paced healthcare environment? We are seeking a driven and personable Admissions & Census Coordinator to help grow and support our community by guiding residents and families through the admissions process while building strong referral partnerships.

In this role, you'll be at the center of our growth-connecting with hospitals, families, and community partners to ensure a smooth transition for new residents while helping us maintain a strong and stable census.

What You'll Do

  • Lead the admissions process from initial inquiry through move-in, ensuring a seamless and positive experience
  • Manage census development goals, including admissions, discharges, and occupancy tracking
  • Verify insurance/benefits and coordinate with internal teams for efficient placement
  • Build and maintain referral relationships with hospitals, physicians, and community partners
  • Generate and track leads, analyze trends, and report on census activity
  • Represent the community at networking events, outreach efforts, and marketing initiatives
  • Communicate effectively with residents, families, and interdisciplinary team members
  • Maintain accurate documentation, reports, and compliance with all regulatory standards
  • Support surveys, audits, and continuous quality improvement efforts

Education & Experience

  • Clinical background preferred: RN, LPN, or Social Work
  • Associate or Bachelor's degree preferred
  • Experience in healthcare admissions, case management, or census development is a plus

Licensure

  • Active nursing license (preferred)
