Care Coordinator

The Care Coordinator will be responsible for linking clients to medical services, assisting clients with navigation of medical services, assisting with addressing barriers to care for both behavioral health and medical care, monitoring client's use of appropriate medical services, and following up with clients on issues relevant to receiving proper medical care. This position will be providing services in the clinic, in the community, and in client homes. Must have knowledge about the medical care system or have a relevant background to be able to attain such knowledge.

Required Education and Experience
  • Bachelor's degree in Psychology, Social Work, Public Health or related field
  • Valid driver's license and proof of insurance
Preferred Education and Experience
  • Experience providing case management services
  • Master's degree in Psychology, Social Work, Public Health or related field
Essential Functions

  • Collaborate with medical and clinical staff as well as conduct client medical needs assessments to determine medical needs of clients
  • Assist clients in arranging medical care by assisting with phone calls and information exchange, providing clarification of medical system terms to clients, advocating with medical providers and payors for proper care
  • Assist with arranging transportation to medical care when needed
  • Assist with arranging childcare, pet care, or other needs when necessary for accessing medical care
  • Become familiarized with the network of medical providers in the area
  • Track appointments, medication, and other medical needs for clients on the Care Coordination caseload as needed
  • Assist with access to housing (search for housing, applications, financial assistance, communication with landlord) as necessary when housing is an issue that is significantly destabilizing with respect to receiving appropriate medical care
  • Assist clients with community resources, public assistance, and benefits
  • Assist client's wellness activities such as health eating, physical activity, and healthy behaviors
