Day Shift | Monday–Friday | 8:00 AM–5:00 PM | No Call

We're seeking a reliable and detail-oriented Licensed Vocational Nurse (LVN) to join a Home Health team as a Clinical Intake Coordinator. This is a weekday, daytime role ideal for LVNs experienced in clinical coordination, referral processing, and scheduling within a Home Health environment.

Position Highlights

Role: LVN Clinical Intake Coordinator Setting: Home Health (Clinical Intake / Referrals) Shift: Day Shift Schedule: Monday–Friday, 5×8s Hours: 8:00 AM–5:00 PM Pay: $1,120 per week Call: None

Role Focus
  • Process and coordinate Home Health referrals
  • Schedule patient start-of-care and follow-up visits
  • Review and manage intake documentation
  • Collaborate with clinical and administrative teams
  • Ensure accurate, timely referral workflow
  • Maintain confidentiality and compliance with regulations
Required Qualifications
  • Active LVN Certification (Required)
  • At least 1 year of LVN experience
  • Experience in Home Health intake, referral processing, or clinical coordination preferred
  • Strong organizational, communication, and multitasking skills
  • Computer proficiency required
  • Ability to work independently in a fast-paced office setting

  • Strong professionalism and attention to detail
  • Must be at least 18 years of age
