STNA / Scheduler McGregor PACE (Program of All-Inclusive Care for the Elderly)

McGregor PACE is a community-based program that provides comprehensive, in-home healthcare services to older adults as an alternative to nursing home placement—empowering seniors to remain safely and independently in their homes.

Position Summary

We are seeking a highly motivated and dedicated STNA to join our team. This is a dual-role position that combines direct participant care in our Adult Day Center with coordination and administrative support for home care services. This role is ideal for someone who enjoys both hands-on care and behind-the-scenes coordination.

Key Responsibilities
  • Provide personal care, assistance with ADLs, and ensure participant safety in the Adult Day Center
  • Greet and assist participants upon arrival, creating a welcoming and supportive environment
  • Support implementation of care plans and healthcare protocols
  • Provide front desk/reception coverage as needed
  • Serve as a liaison between participants, home care agencies, and internal teams
  • Communicate scheduling updates and address service-related concerns
  • Maintain and organize home care documentation, records, and data tracking
  • Collaborate with care teams to ensure participant needs are met
  • Attend IDT meetings as needed to communicate participant care needs
  • Provide in-home support as a Home Health Aide on an as-needed basis
Qualifications
  • High School Diploma or equivalent (required)
  • HHA, CNA, or STNA certification (preferred)
  • Minimum of one (1) year experience working with a frail or elderly population (required)
  • Strong organizational skills and attention to detail
  • Excellent communication and interpersonal skills
  • Basic computer skills, including database and word processing proficiency
  • Ability to build collaborative relationships with internal teams and external partners
