Call 1-800-377-2080 to schedule fingerprint appointments. Give the following number to the fingerprint facility ORI # SB1020006 (Employer Identifier #). Must take written test for CDL driver's license in the following order: Sections 1, 5, 4, 10, 12, 13, 14 (Introduction to Illinois CDL, Air Brake Test, Transporting Passengers Test, School Bus Endorsement Test, Pre-Trip Vehicle Inspection Test, Basic Vehicle Control Skills Test, Drive/Road Testing) at a Secretary of State facility. Take letter of intent for employment from unit office or director of transportation office. And birth certificate. Declare EI when asked about medical certification.
